Instructions for Filing a Complaint


1. Before you file a complaint, read any relevant Consumer Tips. This information is designed to help consumers resolve disputes and protect themselves from future problems.


2. This form cannot be filed electronically. You must either enter the information on the Consumer Complaint form through your keyboard and print the completed form, or print a blank form and complete it in ink.

If you do not have access to a printer, please call the Consumer Protection Division and request a Consumer Complaint form, or mail us the required information in the form of a letter. You can call us toll-free within North Carolina at 1-877-5-NO-SCAM, (919) 716-6000 from outside of North Carolina, or (919) 716-0058 for Spanish speakers.

3. Please provide all the information requested by the form. Explain your complaint in the "EXPLANATION" section and on additional sheets if necessary. Be sure to tell what happened, when it happened, and where it happened, and be specific about any oral statements the business made to you, especially those which influenced you to deal with the company. Try to be brief but thorough. Also include photocopies (not originals) of any written contract, receipts, warranty, advertisements, or letters that are involved.

6. Be sure to sign your name on the completed form.

7. After we receive your complaint, we will review it, and in most cases, contact the business named in the complaint for its response.
